Beyond Gene Mutations: Nature Study Reveals 16 Distinct Epigenetic Subtypes of Acute Myeloid Leukemia For years, the classification of acute myeloid leukemia (AML) has relied largely on identifying genetic mutations. These discoveries have transformed diagnosis, risk stratification, and treatment selection. However, patients with seemingly similar genetic profiles can still experience very different clinical outcomes. A new study published in Nature suggests that part of the answer may lie beyond DNA mutations, in the way the genome is organized and regulated. Researchers analyzed the chromatin landscape of more than 1,500 patients with newly diagnosed AML, creating one of the largest epigenomic studies ever conducted in this disease. Their findings identified 16 distinct epigenetic subgroups, each with unique biological characteristics, prognosis, and drug response patterns. Cancer is often viewed as a disease driven by gene mutations. But genes represent only part of the story. Cells also contain an epigenetic layer: chemical and structural mechanisms that determine which genes are switched on or off without changing the DNA sequence itself. One important aspect of this regulation is chromatin accessibility, which reflects how easily cellular machinery can access specific genes for expression. Researchers hypothesized that studying chromatin accessibility across a large number of AML patients might uncover biological differences that conventional genetic testing cannot detect. The international research team studied 1,563 patients with newly diagnosed AML from independent cohorts in Sweden and Japan. Using ATAC-seq (Assay for Transposase-Accessible Chromatin sequencing) alongside genomic sequencing, RNA sequencing, DNA methylation analysis, histone profiling, and single-cell multiomics, researchers generated an extensive map of the AML epigenome.
