A Common Heart Finding on MRI May Not Need Blood Thinners After All, Major Study Finds Left ventricular hypertrabeculation, previously known as LV noncompaction, has long carried a reputation for danger, historically linked to heart failure, dangerous arrhythmias, blood clots, and death. This reputation led many doctors to recommend long term blood thinners for patients with this finding and reduced heart function, even without atrial fibrillation. A major new international study published in Circulation suggests this long standing practice may need to change. Why this question matters right now: Advances in cardiac MRI have dramatically increased how often hypertrabeculation is detected, in some studies, in up to 43% of healthy people and up to 36% of patients with dilated cardiomyopathy. This surge in detection has raised genuine concern about overdiagnosis and overtreatment, especially since the actual prognostic significance of this finding has remained poorly defined. The study: Researchers analysed 1,160 patients with dilated cardiomyopathy across 22 international centres in the Netherlands and Spain, all evaluated with detailed cardiac MRI, and 86% of whom also underwent genetic testing. Hypertrabeculation was identified using two established methods, showing strong agreement between them, in roughly 30% of patients. Patients were followed for a median of 5.1 years.
